In order to be ready for the school year, please follow the instructions and fill out all forms! If you have any questions please email Jessica Wilder at jessica.wilder@acacademy.com.
1) Read over the handbook, fill out the last 2 pages and email them back to Jessica.
2) If you have not done so already, please fill out the emergency contact form and email it back to Jessica.
3) If your child has a food allergy, please complete the form and email it back to Jessica.
4) The Medical Report can be filled out by your child's doctor but is a required form we must have on file.
